DescriptionHCR ManorCare provides a range of services, including
skilled nursing care, assisted living, post-acute medical and
rehabilitation care, hospice care, home health care and
rehabilitation therapy. Coordinates nutritional care of residents
by completing nutritional assessments, developing and implemeting
care plans, and documenting dietary information about residents. In

Technology with clinical emphasis or Bachelor's degree in nutrition
and dietetics.Registration with the Amercian Dietetic Association
desirable.Position RequirementsOne to two years in related dietary
capacity.Job Specific Details:Job Summary Coordinates nutritional
care of patients by completing nutritional assessments, developing
and implementing care plans, and documenting dietary information
about patients. Refers patients to the Registered/Licensed
Dietitian as appropriate. Essential Job Functions Every effort has

Ensures protected health information is kept confidential ? Reports
complaints made by patients to supervisor ? Reports all allegations
of patient abuse, neglect and/or misappropriation of patient
property Safety ? Uses Standard Precautions as outlined in safety
policies and procedures ? Observes safety needs of patients as
indicated in care plan ? Wears and/or uses safety equipment and
supplies when indicated and properly trained to use ? Demonstrates
job-specific knowledge of fire and disaster preparedness during
drills or actual situations ? Identifies location of MSDS for
chemicals used. Is familiar with eye wash station Staff Development
? Attends and participates in scheduled in-service training,
educational classes and meetings as required by regulatory agencies
and company policies ? Participates in QAA program ? Attends and
participates in Circle of Care classes ? Maintains registration as
required by the Commission of Dietetic Registration when
appropriate ? Provides in-service training, educational classes,
and other opportunities for staff to meet regulations and for
educational development Nutritional Assessment Responsibilities ?
Provides nutritional care within scope of practice under the
direction of a registered dietitian ? Develops and utilizes a
referral system for the registered dietitian for patients at
nutritional risk ? Uses HCR ManorCare forms and guidelines for
nutritional care of patients in the Dietary Procedures Manual ?
Clinical Nutrition Guidelines ? Ensures physician?s orders are
followed making recommendations for changes as needed ? Completes
nutritional assessments, MDSs, RAPs and care plans using electronic
systems ? Provides routine and change of condition progress notes
in the patients? medical records ? Participates with health care
team concerning patients? plan of care during care conferences ?
Checks patients? progress toward meeting nutritional goals and
determines effectiveness of nutritional care plan; documents
outcome in progress notes and adjusts care plans as needed ? Visits
patients regarding menus, food service, food preferences and
nutritional care ? Uses and keeps current company electronic
systems for patient dietary information ? Promotes and assists in
implementation of liberalized diets ? Reviews weight records
routinely and communicates variances to registered dietitian and
interdisciplinary teamNutritional Assessment Responsibilities,
continued ? Communicates patient nutrition information using
24-hour Clinical Report, Eagle Room, SBAR tools and other center
communication systems ? Adheres to HCR ManorCare ?Documentation
Guidelines for the Clinical Record? ? Participates in Quality of
Life programs ? Ensures compliance with nutritional standards,
government regulations, and center policies and procedures ? Uses
sound professional judgment and standards of professional practice
at all times; brings to the center information on current research
and trends in the profession Systems Responsibilities ? Maintains
and uses electronic system for dietary information and tray ticket
printing ? Utilizes PCC and other center electronic systems
appropriately Communication Responsibilities ? Communicates with
the registered dietitian, nursing service, dietary personnel and
other disciplines concerning patient nutritional care. ? Submits
required reports to administrator as requested ? Conducts periodic
staff meetings Food Safety ? Follows guidelines for food safety in
the HCR ManorCare Dietary Procedures Manual ? Food Service
Management, HACCP, state and federal codes and regulations ?
Provides oversight in the maintenance of standards for safe food
handling following center policies and procedures, local and state
food codes and state and federal regulations ? Maintains required
local or state food safety credentials where appropriate ?
Coordinates with food service director and registered dietitian to
conduct safe food handling training of dietary staff ? Performs
regular audits of dietary services to ensure safe food handling
Meal Services/Menus Responsibilities ? Promotes customer service
and patient enjoyment of meals and dining ? Advocates for patients
to honor their choices in food and dining ? Works with food service
director to coordinates dining room service and meals served in
patient rooms ? Performs quality audits of meals and tray lines to
ensure attractive, palatable foods are served at the correct
temperatures ? Makes meal rounds to observe meal service and
solicit patient input concerning food and dining ? Insures the
correct processing of diet order changes and new diet orders from
nursing service ? Works with food service director to ensure that
patients receive correct diets and that food preferences are
honored ? Adjusts menus for patient/regional preferences; approves
the use of nutritionally equivalent substitutes when required ?
Writes menus for non-standard diets Administrative Responsibilities
? Coordinates work with other departments ? Attends and
participates in staff meetings, departmental head meetings, and
other center meetings and sits on required committees ? Assists in
developing and updating departmental procedures and policies ?
Follows HCR ManorCare policy and procedure manuals and programs ?
Uses HCR ManorCare forms ? Performs any miscellaneous work
assignments as may be required Education ? Associates degree in
diet technology with clinical emphasis or Bachelors degree in
nutrition and dietetics ? Registration with the American Dietetic
Association Commission on Dietetic Registration is desirable
Experience ? At least two years experience in long term care,
