Job Description:
A company is looking for an After Care Coordinator to join their
clinical operations team.
Key Responsibilities
Collaborate daily with the Nurse Navigator during the Transitional
Phase
Support After Care Operations and coordinate patient referrals
Manage clinical tasks and follow-ups within the Electronic Medical
Record inbox and portal platform
Qualifications
2 years of previous clinical care experience
High school diploma or GED
Ability to work evenings, early mornings, and/or weekends
Organized and able to thrive in fast-paced environments
Basic knowledge of medical terminology
